Indications for Iron
We follow Calgary Day Medicine Guidelines for iron infusion: women with hemoglobin less than 120 g/dL, or men with hemoglobin less than 135 g/dL.

Please ensure the following labs have been done within 30 days of the Therapy Plan Request: CBC / Iron / Ferritin.
Type of Iron Requested and Special Authorization
If these fields are not completed, we may not be able to enter your Therapy Plan Request in a timely manner.
Please review with the patient whether they have private drug coverage to pay for Monoferric, or, if they are over the age of 65, complete an Alberta Blue Cross Special Authorization form.
If they do qualify for Monoferric (Ferric derisomaltose), please provide them with a prescription to collect at their pharmacy and bring to the infusion appointment at Day Medicine.
There is no charge for adult Day Medicine Infusion appointments, nor for Iron Sucrose, if patients meet the criteria set by Alberta Health Care and Day Medicine-specific policies. Failure to show for appointments may result in cancellation of the plan and any remaining infusions of iron.
