If you need a therapy plan entered for someone in Connect Care but have no access to the system, no Adult Day Medicine privileges, or find the whole process too confusing, you've come to the right place.

Who don't have Connect Care access but a patient needs a plan entered.
Who don't hold Adult Day Medicine privileges and can't submit the plan themselves.
Who don't enter therapy plans regularly and find the process confusing or time-consuming.
How it works

You submit the request

Fill in the therapy plan request and fax the completed form to us at (403) 879-5049.

We contact the patient

We complete and submit the plan in Connect Care, and Day Medicine books the treatment.

Day Medicine infusion

The patient receives their treatment at their preferred Day Medicine site. Our team answers any questions related to the Therapy Plan, and manages infusion reactions.

After the treatment

Patients return to your office for ongoing management. You may send another request for repeat Therapy Plan entry at any time.

Before you send a request

Plans we're unable to enter

Unfortunately, due to the complexity of certain therapy plans and Day Medicine guidelines, we cannot enter the following:

  1. Phlebotomy: we suggest referral to Hematology or Internal Medicine if they do not qualify for blood donation at Canadian Blood Services. Day Medicine does not allow phlebotomy for those on testosterone therapy.
  2. Generic therapy plans for IV fluid and electrolyte infusions (such as for POTS, or functional and eating disorders).
  3. Blood product transfusions, including IVIG: we suggest referral to Internal Medicine, Hematology, or Oncology.
  4. Biologic therapies for Inflammatory Bowel Disease: we suggest referral to Gastroenterology.
